What Should I Fill Out?
Please see below what paperwork you need to accept your appointment. Turn in your completed paperwork in to Isabel Blanco in 6189 Etcheverry Hall in a timely manner. Late paperwork may result in a later hire date than indicated in your letter as a student cannot begin his/her duties without being properly hired.
All GSIs and Readers
- Supplemental GSI Financial Support Form
- GSI Fee Remission Form (for GSIs only)
- Reader Fee Remission Form (for Readers Only)
- Description of Duties Form (for GSIs only)
- Description of Duties Form (for Readers only)
- PLEASE READ Disclosure of Information from Student Records: A Quick Reference for Faculty and GSIs
- PLEASE READ Workers' Compensation Pamphlet: Time of Hire and SIGN the Instructions & Acknowledgement Form
- PLEASE READ the following documents regarding the UC Policy on Substance Abuse to comply with Federal Drug Free Workplace Act:
- Nonresidents also need the following documents:
- Passport Copy
- Visa Copy
- I-20 or DS 2019
- For J-1 Visa Holders: Work Authorization Form from the Berkeley International Office
Continuing GSI and/or Reader with No Break in Service
Also submit a signed Appointment/Offer Letter. Letters are approved by the ME Student Services Office via email. Please only bring the signature/duty page. (Summer is considered a break.)
